How to Report a Change of Name

Marriage:
If your name has changed due to marriage, you will need to send a copy of your marriage certificate.

Divorce:
If your name has changed due to divorce, you will need to send a copy of your divorce decree.

Court Proceedings for Change of Legal Name:
If your name has changed due to court proceedings to legally change your name, you will need to send a copy of the court order of legal name change.
